Retatrutide + Cagrilintide is a weekly research vial of retatrutide (a GIP / GLP-1 / glucagon receptor agonist) and cagrilintide (an amylin analog): 10 mg + 2 mg in a 12 mg vial mixed with 1 mL of water, or 20 mg + 4 mg in a 24 mg vial mixed with 2 mL.

Dosed in insulin units on retatrutide's own weekly steps: 10 units (retatrutide 1 · cagrilintide 0.2 mg), 20, 40 and 80 units, topping out at 85 units (8.5 · 1.7 mg). It is not CagriSema.

Best for

Weight-management research interest in pairing a triple incretin agonist with an amylin analog. Neither compound is approved, and no human trial has tested the pair.

What to expect

Each compound's trials measured weight change over months (26 weeks for cagrilintide, 48 weeks for retatrutide); the only study of the two together was in rats.

Pairs with

Already an incretin plus an amylin analog: do not add a GLP-1-class drug (retatrutide, tirzepatide, semaglutide) or a second amylin agonist (cagrilintide, eloralintide) on top of it.

Research protocol

Once weekly, under the skin, in the morning. Mix a 12 mg vial with 1 mL of water or a 24 mg vial with 2 mL — either way each unit is 0.1 mg retatrutide + 0.02 mg cagrilintide.

The ladder follows retatrutide's weekly steps: 10 units (1 mg + 0.2 mg), 20 (2 + 0.4), 40 (4 + 0.8) and 80 (8 + 1.6), with 85 units (8.5 + 1.7) as the top, where cagrilintide reaches its own 1.7 mg top. Step up no faster than every 4 weeks, and only if progress has stalled — the ladder is a ceiling, not a schedule.

Specific dosing varies by goal and physician guidance.

CautionNo study has given retatrutide and cagrilintide together to people — only rats (2026). Both slow the stomach and cut appetite, so expect more nausea than with either alone; step up slowly. Dose in the morning (retatrutide can disrupt sleep). This is not CagriSema. A 24 mg vial (20 + 4) takes 2 mL of water, not 1 mL: at 1 mL every unit on this ladder is a double dose. A vial already mixed at 2 mL from a 12 mg vial gives half of each rung’s figures.

How it works

Retatrutide is an investigational single peptide that activates three receptors — GIP, GLP-1 and glucagon; its phase 2 obesity trial gave 1–12 mg once a week for 48 weeks. Cagrilintide is a long-acting amylin analog; its phase 2 dose-finding trial gave 0.3–4.5 mg once a week.

The pairing rests on complementary appetite pathways (incretin and amylin), the same rationale as CagriSema (cagrilintide with semaglutide). The only published study of retatrutide with cagrilintide gave them to obese male rats; no human study exists, and neither compound is approved.
