How it works

SS-31 (elamipretide) is a four-amino-acid Szeto-Schiller peptide that concentrates in the inner mitochondrial membrane and binds cardiolipin, where research describes stabilized electron-transport-chain supercomplexes, improved ATP production and reduced reactive-oxygen-species leak. MOTS-c is a 16-amino-acid peptide encoded in mitochondrial DNA that activates AMPK and, under metabolic stress, translocates to the nucleus to regulate stress-response genes; animal studies report improved insulin sensitivity and exercise capacity. The two are paired on the rationale that one acts on the membrane machinery and the other on metabolic signaling.

Elamipretide has been through human trials (including Barth syndrome); MOTS-c evidence is animal and early human. Neither is FDA-approved for these uses; the blend is handled as a research compound.

Research protocol

Subcutaneous injection every other day, morning or pre-workout, dosed in units from a 50 mg vial reconstituted with 3 mL of bacteriostatic water. 8 weeks on, 4 weeks off — both components are typically cycled rather than run continuously.

What to expect

MOTS-c studies report metabolic and exercise-capacity changes over weeks; elamipretide trial endpoints were measured over weeks to months. Human onset data for the blend itself do not exist.
