How it works

PT-141 (bremelanotide) is a synthetic cyclic heptapeptide melanocortin receptor agonist, structurally derived from the alpha-melanocyte-stimulating hormone (alpha-MSH) analog melanotan II via a lactam bridge that stabilizes its cyclic conformation. Unlike PDE5 inhibitors that act peripherally through the nitric-oxide/cGMP vasodilatory pathway, bremelanotide acts centrally, nonselectively activating melanocortin receptors (notably MC4R, and MC3R) in hypothalamic and limbic circuits that research associates with regulation of sexual desire and arousal. Phase 3 research in premenopausal women characterized its effects on self-reported sexual desire and associated distress, and functional MRI studies have explored its central activity.

Bremelanotide is FDA-approved as Vyleesi (2019) for acquired, generalized hypoactive sexual desire disorder (HSDD) in premenopausal women; other uses, including in men, are not FDA-approved and remain investigational or off-label research applications.

In research and the FDA-approved Vyleesi product it is given subcutaneously on an as-needed basis ahead of anticipated activity (the approved product dose is 1.75 mg, while compounded research protocols often reference smaller microgram-range doses), with sources cautioning against more than one dose per 24 hours.
